I have the Vuka race extensions, I love the hand position on those; probably my favourite over the last 25 years.

Personally, I can hold that head position for long periods; I find it hard on the neck & shoulders though and I always look forward to a sports massage to ease things up. A long tailed helmet that mates well with your back will help in any case.

I can hold an agressive turtle position for a 10, and just about a 25 if it's a standalone TT. However, if I was doing a tri or a 50, I'd take every opportunity (steep hills/tight corners/roundabouts etc) to stretch out a bit and change position
